A 40 year old male presented with 15 day history of

  • Headache.
  • Double vision.
  • Blurring of vision.
On examination:
  • Bitemporal hemianopia (decreased vision on the sides).
  • Left lateral rectus palsy (inabity to look left  side with left eye) with new squint.
  • Decreased vision in both eyes.
 
MRI brain showed  a tumour in the region of pituitary gland along with invasion of the cavernous sinus. 
 
The patient had an immediate improvement in the vision. The squint has resolved completely.
